Registration - MyAP®

To access and useMyAP®, through Care Track®, agreement to the Terms and Conditions, Registration and Payment of a small non-refundable fee are required.

  1. Agreement to Care Track® -MyAP® Terms and Conditions of use is required prior to submitting registration. See “tick box” in Registration box below.

  2. Registration consists of submitting an email address and a strong password.

    There is no provision for submitting a name, residential address, age, gender, race, religion, or the like anywhere in theMyAP® and this may be considered in the wording in the email address to distance a person from an email address. The email address submitted will be rejected if it has been previously used inMyAP®.

    The password should be at least 8 characters in length and a combination of letters and numbers may be considered. The password may be stored elsewhere safely.
  3. Registration will be confirmed by email to the email address submitted for registration. This email address will be forever linked to the Personal Identification Number ( PIN ) that has been allocated and included in the email.

    The PIN (with the password submitted below) will be used to LOGIN toMyAP®.

    The email address will only be used by Care Track, from this point, for communication purposes. The email will contain further information regarding payment.
  4. Go to the “Register here” box below, type in the email address, password and confirm password. Agree (or not) to the Terms and Conditions of use.
  5. Click on / touch the “Register” button to complete registration.
